Challenge
In 2019, a fall-from-elevation event at a large electric utility prompted enterprise corrective actions on tool safety. The root cause identified three significant contributors which included inadequate training, inability for supervisors to verify safe work practices using specific tools, and individuals’ inabilities to assess and mitigate risks. Additionally, there was no clear procedural guidance in place on how to use high-risk tools in the field.
Process
- Assessed the risk of more than 100 tools across three operating units based on inherent tool risk, frequency of use, and situational
hazards - Identified 55 high-risk tools with significant safety concerns
- Developed a comprehensive high-risk tool program, including standardized procedures
- Created NXT GEN® Training modules incorporating first-person perspectives and interactive procedural guidance on proper and improper tool use
- Engaged more than 100 utility subject matter experts to ensure training accuracy and relevance
- Integrated NXT GEN® Training into the existing learning management system
- Trained supervisors on workforce validation to ensure proper tool use through performance-based assessments
- Incorporated confidently held misinformation theory to correct common safety misconceptions in training
Result
- Established an enterprise-wide program for high-risk tools, improving knowledge sharing and leading to tool replacements for equipment
with inadequate safety features - Developed 25 computer-based training modules on situational risk, hazard mitigation, and avoidance, training more than 1,200
supervisors and technicians - Launched a mobile worker platform with 20 just-in-time training modules
- Built a reference library of NXT GEN® Training videos and simulation content for on-demand access
- Developed and updated multiple operating procedures for 35 high-risk tools
