In 2019, a fall-from-elevation event at a large electric utility prompted enterprise corrective actions on tool safety. The root cause identified three significant contributors which included inadequate training, inability for supervisors to verify safe work practices using specific tools, and individuals’ inabilities to assess and mitigate risks. Additionally, there was no clear procedural guidance in place on how to use high-risk tools in the field.
