Inconsistent procedures across four operating regions led to variations in power factor testing, qualification requirements, and contractor oversight. Field observations with utility crews in Generation, Nuclear, and Transmission mirrored blue-line assessments, capturing real-time work execution and procedural gaps. Key risks included inconsistent MAD adherence, informal validation of contractor work, and variations in safety enforcement.
