Challenge
In 2019, a fall-from-elevation event at a large electric utility prompted enterprise corrective actions on tool safety. The root cause identified three significant contributors: inadequate training, supervisors’ inability to verify safe work practices using specific tools, and individuals’ inability to assess and mitigate risks. Additionally, there was no clear procedural guidance on using high-risk tools in the field.
Process
- Assessed the operational risk of 100+ tools across three operating units based on inherent tool risk, frequency of use, and situational risk
- Identified more than 55 potential high-risk tools that have a significant safety risk
- Developed high-risk tool program, including procedures
- Developed NXT GEN® Training programs, which leverage first-person view along with interactive procedural guidance and content regarding what to do vs. what not to do
Engaged more than 100 utility SMEs in the development of the training - Integrated NXT GEN® Training into existing learning management system
- Trained supervisors on how to validate that their workforce is safe to work with the select tools through performance verifications
- Introduced confidently held misinformation theory into the learning curriculum
Result
- Established a standard enterprise program for high-risk tools, which improved peer sharing of leading practices and led to tool replacements for equipment with improper safety features
- Developed 25+ computer-based training modules on situational risk, hazard mitigation, and avoidance. Trained more than 1,200 first-line supervisors and technicians
- Developed a mobile worker platform with more than 20 just-in-time training modules
- Built a reference library of NXT GEN® Training video and simulation content for on-demand use
- Developed or updated multiple operating procedures for more than 35 high-risk tools
